New Paid Order with Free Materials Questions
  1. What do I do if I cannot find the form for the program I am looking for?

    Contact HMH Sales Support at 1-800-426-6577 and they will provide the gratis order form you require.

    Please Note: Gratis forms for pre-2009 adoptions are not posted on the website, but can be requested through Sales Support.

  2. What documents are needed for my proof of purchase?

    A critical part of completing your free materials request includes faxing an acceptable proof of purchase document along with the fax cover sheet provided during the online order process.

    The following are acceptable proof of purchase documents:

    • The purchase order or requisition sent to your district, state or depository for the sales items purchased this year.
    • The invoice from the state or depository for the sale items purchased this year.
  3. What information do I need to have in order to complete the request?

    You need to have documentation showing the number of student editions purchased for each grade level in order to complete the request.

  4. What if the sales order was shipped to my district and I need the free materials sent to my school?

    We are more than happy to ship your free materials to the school. Please complete the request on the website and provide documentation showing the number of student editions you asked to be purchased by the district.

Repeat Consumables Order Questions
  1. What do I do if I cannot find the form for the program I am looking for?

    Contact HMH Sales Support at 1-800-426-6577 and they will provide the gratis consumables re-order form you require.

  2. When may I submit a request to refill my consumables?

    The consumables can be requested each year beginning the 2nd year of the adoption through the life of the adoption.

  3. How do I determine the quantity of consumables that I can request?

    We will fill consumable requests at the same ratio as the student editions purchased. You should check to see how many student editions were purchased for each grade level before submitting a request through this website.

  4. What if the 1st year adoption order was sent to a district and I need my refill consumables sent to my school?

    In this case, the district will have to request workbooks. Because the order history is at the district address, the repeat workbooks will have to be ordered through the district in order to check the total quantities and ISBNs. Your district can send in a request to ship to the individual schools, but they must place the entire refill order request at one time.
